* Proof of status is required (A proof of status is an official letter written in English by the head of the hospital department on official hospital letterhead)
**A copy of the press card dated 2008/2009 or the original assignment letter is required
- Membership status (payment dues paid up to and including 2009) will be checked by the EAU Central Office and the online registration system.
- The online registration system will close on 27 February 2009. After this date registrations can be made onsite at the congress venue from 17-21 March 2009.
WHAT IS INCLUDED?
Delegates are entitled to:
- participate in all scientific sessions
- attend the Opening Ceremony and Welcome Cocktail
- attend the technical exhibition
- obtain all congress documents/ congress bag
- obtain a public transportation pass
Accompanying persons are entitled to:
- attend the Opening Ceremony and Welcome Reception
- attend the EAU Evening
- attend the technical exhibition
- obtain a public transportation pass
Exhibitors are entitled to:
- attend the Opening Ceremony and Welcome Reception
- attend the technical exhibition
- attend the satellite sponsored symposia (all satellite symposia, not only of their own company!)
Press is entitled to:
- attend the Opening Ceremony and Welcome Reception
- attend the technical exhibition
- attend the scientific sessions
- obtain a press congress bag (= same as normal congress bag but they have to retrieve it at the press centre)

CANCELLATION POLICY
(fees are excl. 25% VAT)
- Before 12 January 2009: € 50 will be withheld
- Between 12 January and 12 February 2009: 50% refund
- After 12 February 2009: no refund (cancellation and new registration)
All refunds will be handled after the congress

Please note that there are special cancellation and name change arrangements for gold and platinum sponsors of the 24th Annual EAU Congress.
TERMS OF PAYMENT FOR REGISTRATION FEES, ESU COURSES AND THE EAU EVENING
- All payments must be made in Euro
- Please note that you will be charged a higher fee when we receive payment after the stated deadlines.
- Payments can be made by credit card (secure payment page), cheque or bank transfer
- Registration will only be valid upon receipt of the full payment by the EAU Central office. A confirmation and invoice confirming registration will only be sent after receipt of the appropriate fees
- Outstanding payments must be paid on-site. A copy of the bank transfer (or other proof of payment) will be required in the event that registration fees were not credited to the congress bank account on time.
- All refunds will be handled after the congress
By cheque
Cheques must be in Euro, made payable in the Netherlands and made out to order of Congress Consultants B.V. EAU Stockholm 2009. All cheques must be sent with details of registration reference number and delegate's name to ensure payment is allocated to the correct registration.
