EAU Gold/ Platinum sponsors
Please note that there are special registration conditions for gold and platinum sponsors of the 24^th Annual EAU Congress. You are kindly requested to contact the registration department at registrations@congressconsultants.com.

Please read the following instructions carefully before making a group registration

• Please note that the online group registration can only be used to register congress delegates.
• Group registration is only possible for companies or travel agencies wishing to register delegates for the congress.
• Registrations will not be accepted without the work or home address of each delegate.
• It is not possible to register an accompanying person without a delegate.

HOW TO MAKE A GROUP REGISTRATION ONLINE:

In order to register your delegates for the congress you need to log in on the online registration system with your username and password. If you never requested or received a password from the EAU before, you can request a username and password on the first page of the online registration system. After having received your username and password you can start registering your delegates.
Choose "Another/ Multiple persons" if you wish to register 1 or more persons (other than yourself).

FEE REDUCTIONS

• EAU member:
  The EAU member registration fee will automatically be calculated by the online registration system. The online registration system will alert you if membership fee payment is due. In this case please contact the membership department: membership@uroweb.org
• EAU junior member / EAU junior international member:
  The EAU junior member / EAU junior international member registration fee will automatically be calculated by the online registration system. The online registration system will alert you if membership fee payment is due. In this case please contact the membership department: membership@uroweb.org
  After completing your registration you will be sent a confirmation together with an invoice for any extra fees that need to be paid. The invoice can be filled in with credit card details and faxed back to the Congress Consultants, fax: +31 26 389 17 52
• Ph.D. Researcher EAU affiliated member:
  The Ph.D Researcher EAU affiliated member registration fee will automatically be calculated by the online registration system. The online registration system will alert you if membership fee payment is due. In this case please contact the membership department: membership@uroweb.org. A proof of status is required to confirm your status as Ph.D Researcher. Without a proof of status, reduction on the registration fee will not be granted.
• Non EAU member
  If you are a resident, PhD Researcher or a nurse; please send a proof of status to Congress Consultants, the EAU Congress Office. This does not apply to members of the EAU or EAUN. A proof of status is an official letter written by the head of the hospital department on official hospital letterhead. Without a proof of status, reduction on the registration fee will not be granted.

REGISTRATION DEADLINES

• Early:
  12 January 2009: deadline payment early registrations fee (payment should have been received by the EAU)
• Late:
  12 February 2009: deadline payment late registrations fee (payment should have been received by the EAU)
• Online registrations:
  27 February 2009: deadline online registrations. After this date registrations can be made on-site at the congress venue from 17-21 March 2009.

COLLECTION OF CONGRESS REGISTRATION DOCUMENTS

Collective pick-up (by a representative of your company/ travel agency)
When you register a group and wish to pick up your delegates' congress documents, we strongly suggest that you make an appointment on 15 or 16 March, since 17 March will be very busy and no appointments can be made. Please contact the registration department (phone: +31 26 389 1751) if you wish to make an appointment on Monday 15 or Tuesday 16 March (the group desks will be open from 08.00 till 20.00).

If no appointment has been made, your representative can only collect your delegates' congress documents upon presentation of the confirmation at the group registration desk on 17-21 March 2009.

Individual pick-up (by the delegate)
If your delegates are going to collect their congress documents individually at the pre-registration desk, you will receive a separate confirmation for each delegate. Please make sure your delegates receive these confirmations. Congress documents will only be handed out upon presentation of a registration confirmation.
